Billing and Coding: MolDX: Molecular Diagnostic Tests (MDT)

This policy specifies billing and coding instructions for reporting MolDX Molecular Diagnostic Tests: select the appropriate CPT code, enter 1 unit of service, and include the DEX Z-Code identifier adjacent to the CPT code in the claim comment/narrative field. The policy defines exact claim fields for placement of the DEX Z-Code (Part B electronic: Loop 2400 or SV101-7; Part B paper: Box 19; Part A electronic: SV202-7; Part A paper: Block 80) and prohibits adding extra characters on SV101-7 for Part B claims.
