CPT codes 96132–96139 cover neuropsychological testing intended to diagnose or characterize neurocognitive effects of medical disorders affecting the brain and must be documented as necessary for diagnosis or therapeutic planning. Documentation must be legible and support selected ICD-10-CM and CPT/HCPCS codes, include tests performed with scoring/interpretation and time for time-based services (typically 4–6 hours), and if testing spans multiple days total time must be combined and reported on the last date of service; do not bill these codes for services that rely primarily on self-report questionnaires, rating scales, or projective techniques.
