Billing and Coding: Independent Diagnostic Testing Facility (IDTF)

This policy covers diagnostic testing performed in Independent Diagnostic Testing Facilities only if the facility follows CMS supervision/credentialing rules and lists the exact CPT/HCPCS codes and equipment on the CMS-855B (Attachment 2, Section 1); claims for procedure codes not reported on the CMS-855B will be denied. Key requirements: IDTFs must meet supervising physician and technician qualifications at enrollment, undergo desk and mandatory site reviews for new applicants, amend the CMS-855B to add additional similar codes (no new site visit for similar codes) while adding different procedure types or supervision levels requires a new site visit, and obtain contractor approval to bill each code.
