This policy provides coding guidance for MIGS: use CPT 66991 for noncomplex cataract removal with IOL plus concomitant intraocular aqueous drainage device, CPT 66989 for complex cases, and CPT 65820 for goniotomy (per eye), but goniotomy must not be reported in addition to other angle surgeries, stent insertions, Schlemm canal implants, or when the incision is minimal/incidental. Key requirements: follow NCCI edits and related LCD limitations, use the specified claim fields for Part A (SV202-7/Block 80) and Part B (Loop 2400/SV101-7/Box 19), and for bilateral Part B surgeries submit one line with modifier -50 per MUEs.
