Billing and Coding: MolDX: Plasma-Based Genomic Profiling in Solid Tumors

This billing/coding guideline complements LCD L38065 for Plasma-Based Genomic Profiling in Solid Tumors and requires selecting the appropriate CPT and ICD-10-CM codes. Claims must report exactly 1 unit of service and include the appropriate DEX Z-Code identifier adjacent to the CPT code in the specified comment/narrative fields for Part B (Loop 2400 or SV101-7; Box 19 for paper) and Part A (SV202-7; Block 80 for UB04) claims; do not add extra characters on SV101-7.

"Enter exactly 1 unit of service (UOS) per claim for a Plasma-Based Genomic Profiling in Solid Tumors service; do not report multiple units on the same claim."
