This billing and coding policy covers CPT codes 64582 (implantation), 64583 (revision/replacement), and 64584 (removal) of hypoglossal nerve stimulation systems for treatment of obstructive sleep apnea when reasonable and necessary per LCD L38310. Do not report these CPT codes together in the same encounter as specified, use modifier 52 for reduced services involving partial component revision/removal, and comply with NCCI edits. Complete, legible medical records with provider signature and documentation supporting the diagnosis codes must be maintained and made available upon request.
