This guidance specifies billing and claim-entry requirements for the MolDX Pigmented Lesion Assay: report the appropriate CPT and ICD-10-CM codes, include the DEX Z-Code adjacent to the CPT code in the specified comment/narrative fields for Part A and Part B claims, and enter 1 unit of service.
