Policy Summary
Non‑invasive FFR (FFRct) is a post‑processing service covered when performed following and based on a previously performed CCTA for evaluation of suspected stable ischemic heart disease, with the CCTA fully reviewed and its report submitted. Claims must include the referring physician's name and NPI and the medical record must document clinical findings leading to CCTA, symptoms consistent with stable ischemic heart disease, BMI, and the FFR analysis report; billing is subject to NCCI/OPPS edits and services likely to be non‑covered may require an ABN.
