Billing and Coding: Implantable Continuous Glucose Monitors (I-CGM)

This billing and coding article designates CPT/HCPCS codes for insertion (0446T), removal (0447T), and removal with immediate replacement (0448T) of implantable continuous glucose monitors (I-CGM) and notes sensors are available as 90-, 180-, or 365-day devices. Coverage requires that the device and procedure meet the reasonable and necessary criteria in LCD L38623; full, legible medical records including provider signature, supporting ICD-10-CM selection, and accurate CPT/HCPCS coding must be maintained and produced upon request. Non-covered services must not be billed as covered and should be billed with the appropriate modifier.
