Codes apply specifically to 90-, 180-, and 365-day implantable sensors, and claims must be supported by legible medical records with patient identification, a provider signature, and documentation that justifies the selected ICD-10-CM and CPT/HCPCS codes; non-covered services per the LCD must not be billed as covered and require use of the appropriate modifier.
