Billing and Coding: Implantable Continuous Glucose Monitors (I-CGM)

This billing and coding article instructs that implantation, removal, and removal-with-reinsertion of implantable continuous glucose monitors (I-CGM) that meet the reasonable and necessary criteria in LCD L38657 are reported with CPT/HCPCS codes 0446T, 0447T, and 0448T respectively. Codes apply specifically to 90-, 180-, and 365-day implantable sensors, and claims must be supported by legible medical records with patient identification, a provider signature, and documentation that justifies the selected ICD-10-CM and CPT/HCPCS codes; non-covered services per the LCD must not be billed as covered and require use of the appropriate modifier.
