This billing and coding article provides CPT guidance for implantable continuous glucose monitors: use CPT 0446T for insertion of 90-, 180-, or 365-day I-CGM sensors, CPT 0447T for removal, and CPT 0448T for removal with immediate replacement. Replacement of sensors (including replacing a 90-day with a 365-day sensor) is allowable for patient discomfort or need for a different anatomic site, end of functional life, or device malfunction; documentation requirements and LCD L38617 reasonable-and-necessary criteria must be met and all records retained and signed by the treating provider.
