Billing and Coding: Implantable Continuous Glucose Monitors (I-CGM)

Medicare covers implantation (0446T), removal (0447T), and removal-with-immediate-replacement (0448T) of 90-, 180-, or 365‑day implantable continuous glucose sensors only when the procedures meet the reasonable-and-necessary criteria in LCD L38659; services that do not meet the LCD are non‑covered and must be billed using the appropriate modifier. All care must be documented in the medical record (legible, with patient identifiers, dates of service, and the responsible clinician’s signature) and made available to the contractor upon request.
