Billing and Coding: MolDX: Molecular Testing for Solid Organ Allograft Rejection

This MolDX billing/coding policy lists molecular assays for solid organ allograft rejection that have completed MolDx technical assessment and are deemed compliant for coverage when the applicable MolDx LCD/policy coverage criteria are met. Claims must include the appropriate CPT and ICD‑10‑CM codes, one unit of service, and the correct DEX Z‑Code identifier placed adjacent to the CPT code in specified Part A/Part B claim fields; covered tests and specimen types are enumerated (heart, kidney, lung; blood or urine) for each validated assay.

"Molecular testing for detection or monitoring of solid organ allograft rejection is covered when the patient and clinical situation meet the coverage criteria of the referenced MolDx LCD/policy."
