Billing and Coding: MolDX: Prognostic and Predictive Molecular Classifiers for Bladder Cancer

This billing and coding policy provides claim-level instructions for MolDX prognostic and predictive molecular classifier testing for bladder cancer (see LCD L38684). Required claim elements include the appropriate CPT and ICD-10-CM codes, entry of the DEX Z-Code adjacent to the CPT code in specified Part A and Part B fields, and reporting one unit of service (1 UOS) per CPT/claim line; do not add extra characters to the SV101-7 line.
