Billing and Coding: MolDX: Blood Product Molecular Antigen Typing

Panel-based blood product molecular antigen typing (PLA panels 0001U and 0084U) is covered but limited to one use per beneficiary lifetime. Individual single-antigen codes (PLA 0180U–0201U, 0221U, 0222U and CPT 81105–81112) are noncovered when billed alone and will be considered only as part of an approved panel; multiple antigens are required for a comprehensive evaluation. Claims must include the correct CPT/PLA code, unit(s) of service, DEX Z-Code in the specified Part A/Part B fields, and appropriate ICD-10-CM diagnosis code(s).
