Policy Summary
Non-invasive FFR (FFRct) is billed as a post-processing analysis of a previously acquired CCTA and is intended for patients being evaluated for stable ischemic heart disease; the CCTA must be fully reviewed and its report submitted before FFRct. Claims must include detailed documentation (clinical findings leading to CCTA, symptoms consistent with stable ischemic heart disease, BMI, the FFR analysis report) and the name/NPI of the referring/ordering physician; billing is subject to NCCI/OPPS edits and an ABN may be used when coverage is uncertain.
