Response to Comments: Percutaneous Vertebral Augmentation (PVA) for Vertebral Compression Fracture (VCF)

This revision clarifies that percutaneous vertebral augmentation (PVA) is covered for vertebral compression fractures (VCF) and explicitly expands coverage to include subacute fractures occurring 6–12 weeks after onset. The policy adds a continuum-of-care approach and removes the prior requirement for multidisciplinary team consensus; administrative effective dates and comment/meeting dates are noted.
