Policy Summary
Non-invasive FFRct is covered only as post-processing analysis of a previously performed and fully reviewed coronary CT angiography (CCTA) to evaluate suspected stable ischemic heart disease; documentation must include the CCTA report, clinical findings prompting CCTA, symptoms consistent with stable ischemic heart disease, BMI, and the FFRct analysis report. Claims must include a valid ICD-10-CM diagnosis code and the referring/ordering physician's name and NPI; billing is subject to NCCI and OPPS edits and non-coverage rules (use ABN when appropriate).
