This billing and coding guidance requires reporting the appropriate CPT and ICD-10-CM codes, entering one unit of service, and including the correct DEX Z-Code identifier adjacent to the CPT code in specified claim fields depending on claim type (Part B 837P Loop 2400/SV101-7 or paper Item 19; Part A 837I SV202-7 or UB-04 Block 80). The policy also explicitly prohibits adding any extra characters or information when entering the DEX Z-Code on the SV101-7 field.
