Billing and Coding: MolDX: Molecular Testing for Detection of Upper Gastrointestinal Metaplasia, Dysplasia, and Neoplasia

This billing and coding guideline complements the MolDX LCD for molecular testing of upper gastrointestinal pathology and requires submission of specific claim elements: the appropriate CPT code, exactly 1 unit of service, the DEX Z‑Code placed adjacent to the CPT code in specified Part A or Part B claim fields, and an appropriate ICD‑10‑CM diagnosis code. Placement details differ by claim type and format (Part B: Loop 2400/SV101‑7 for 837P or Item 19 for paper; Part A: Line SV202‑7 for 837I or Block 80 for UB‑04).
