Billing and Coding: eVox^® System and Other Electroencephalograph Testing for Memory Loss

eVox®, BrainView NeuralScan, VoxNeuro and similar EEG/ERP memory-loss test panels must be billed using the unlisted neurological/neuromuscular CPT code 95999 (one unit) with the device name entered in the Part A Remarks or Part B Narrative—billing the individual CPTs 92650, 92653, 95816, 95930, 95957 or 96146 is incorrect. All documentation must be maintained in the patient's medical record, legible, include patient identification, and made available to the contractor upon request.
