Billing and Coding: MolDX: Molecular Testing for Detection of Upper Gastrointestinal Metaplasia, Dysplasia, and Neoplasia

This billing/coding guidance requires use of the appropriate CPT code with one unit of service and the adjacent DEX Z-Code identifier placed in specified comment/narrative fields for Part A and Part B electronic and paper claims, and the correct ICD-10-CM diagnosis must be reported. Failure to include the required DEX Z-Code adjacent to the CPT code may result in claim denial.
