Vitamin D assay testing is covered when ordered for evaluation of vitamin D status and billed with a valid ICD-10-CM diagnosis that best describes the patient's condition, consistent with LCD L39391. Documentation must justify test selection, include the ordering/referring physician name and NPI when required, and providers must not bill component sources of 25-OH vitamin D separately; repeat testing for clinically stable patients with levels >30 ng/mL is discouraged and requires clear documentation of necessity. Coding must comply with NCCI and OPPS edits prior to billing.
