This billing and coding guidance covers thermal destruction (ablation) of the intraosseous basivertebral nerve (BVN) for vertebrogenic lower back pain and supplements LCD L39420. Use CPT 64628 for the first two lumbar or sacral vertebral bodies with imaging guidance and CPT 64629 to report each additional vertebral body (imaging guidance must be documented); 64629 is billed only in addition to 64628. Claims must document the vertebral levels treated and the use of imaging guidance and adhere to LCD L39420 for clinical coverage and any frequency limits.
