This billing and coding guidance requires reporting the appropriate CPT and ICD-10-CM codes, entering 1 unit of service per claim, and including the DEX Z-Code™ adjacent to the CPT code in specified claim fields. DEX Z-Code placement differs by claim type: Part B electronic (Loop 2400 / SV101-7) or paper (Box 19), and Part A electronic (Line SV202-7) or UB04 paper (Block 80).
