AECG monitoring is covered when the medical record documents a diagnosis or clinical suspicion of a cardiac event and supports the medical necessity and chosen monitor type/duration (e.g., symptoms such as syncope, dizziness, chest discomfort, palpitations, abnormal tests, or cardiac history). Coverage should follow the LCD L39492 and relevant NCD guidance (e.g., NCD 20.15 for post-infarct), adhere to specified CPT code–to-duration mappings, and meet detailed documentation requirements (legible record, identifiers, signature, interpretation). CPT 93228/93229 are limited to one claim per patient per 30 days and non-covered services must not be billed as covered.
