This document is a response to public comments on the MolDX Local Coverage Determination L38586 concerning prognostic and predictive molecular classifiers for bladder cancer and does not itself state clinical coverage criteria. The notice period began 07/06/2023 and the LCD becomes effective 08/20/2023; extractable coverage, limitation, documentation, and frequency criteria must be obtained from LCD L38586 itself.
