Molecular testing for risk stratification of thyroid nodules is covered per MolDX (see LCD L39646), primarily for patients with indeterminate thyroid cytology. The test is expected to be performed once per patient lifetime (a second unrelated indeterminate nodule may be considered with supporting documentation), and claims must include the correct CPT, one unit of service, the appropriate ICD-10-CM code, and the DEX Z-Code in the specified claim fields without additional characters.
