Billing and Coding: MolDX: Molecular Biomarkers for Risk Stratification of Indeterminate Pulmonary Nodules Following Bronchoscopy

This billing/coding guideline requires that the MolDX molecular biomarker test be billed using CPT 81479 with one unit of service and limits billing to one test per patient. The appropriate DEX Z-Code identifier must be entered adjacent to the CPT code in specified electronic and paper claim fields (Part B: Loop 2400/SV101-7 or Box 19; Part A: SV202-7 or UB04 Block 80), and the correct ICD-10-CM diagnosis code must be reported; do not add extra characters on the SV101-7 line.
