Billing and Coding: MolDX: Molecular Biomarkers for Risk Stratification of Indeterminate Pulmonary Nodules Following Bronchoscopy

This billing/coding guideline covers molecular biomarker testing for risk stratification of indeterminate pulmonary nodules obtained following bronchoscopy. Claims must use CPT 81479, bill one unit and only one test per patient, and include the appropriate DEX Z-Code adjacent to the CPT code in specified Part A (SV202-7 or Block 80) or Part B (Loop 2400, SV101-7, or Box 19) claim fields; also report the appropriate ICD-10-CM diagnosis code. When using SV101-7 for Part B, do not add extra characters to the DEX Z-Code field.
