Molecular testing for risk stratification of thyroid nodules is covered when clinical criteria of MolDX LCD L39682 are met; the test is generally allowed once per patient lifetime. A second, unrelated indeterminate thyroid nodule may be considered for coverage with supporting clinical documentation. Claims must include the appropriate CPT code (1 UOS), the DEX Z-Code placed adjacent to the CPT in specified Part A/B claim fields, and the appropriate ICD-10-CM diagnosis code, with no extra characters added on SV101-7 lines.
