Billing and Coding: MolDX: Molecular Testing for Risk Stratification of Thyroid Nodules

MolDX expects molecular testing for risk stratification of thyroid nodules to be performed once per patient lifetime; a second, unrelated indeterminate thyroid nodule may be considered only with supporting documentation. Claims must include the appropriate CPT code, one unit of service (1 UOS), the appropriate DEX Z-Code entered adjacent to the CPT code in the specified Part A/Part B claim fields (respecting SV101-7 formatting), and an appropriate ICD-10-CM diagnosis code.

"Molecular testing for risk stratification of thyroid nodules per MolDX (see LCD L39684) is the intended service."
