Billing and Coding: MolDX: Molecular Biomarkers for Risk Stratification of Indeterminate Pulmonary Nodules Following Bronchoscopy

This billing/coding guidance requires use of CPT 81479 with one unit of service and permits only one molecular biomarker test billed per patient. The appropriate DEX Z-Code must be entered adjacent to the CPT code in specified claim fields for Part A (SV202-7, Block 80) and Part B (Loop 2400/SV101-7 for electronic, Box 19 for paper) claims, and the correct ICD-10-CM diagnosis code must be reported. Do not add extra characters to the SV101-7 DEX Z-Code field for Part B electronic claims.
