KidneyIntelX and KidneyIntelX.dkd testing are covered only when all criteria in the applicable LCD's Indications of Coverage are met and, if met, are reasonable and necessary once in a lifetime. Claims must include a valid ICD-10-CM diagnosis code and, where applicable, the referring/ordering physician's name and NPI; report PLA code 0105U for services 2024-04-01 through 2024-06-30 and PLS code 0407U for services on/after 2024-07-01. Procedure codes may be subject to NCCI and OPPS edits which can affect billing and payment.
