Policy Summary
Levonorgestrel-releasing IUDs can be used to treat abnormal uterine bleeding (including endometrial hyperplasia without atypia), but Medicare treats contraceptive device insertion (CPT 58300) as non‑covered and will auto‑deny payment. To seek reimbursement, providers must bill CPT 58999 (unlisted procedure) with the appropriate diagnosis (e.g., endometrial hyperplasia), include the product description “hormone IUD for endometrial hyperplasia” in item 19 of the CMS‑1500, and ensure the diagnosis justifies the service.
