Policy Summary
Levonorgestrel (hormone) IUD may be used to treat abnormal uterine bleeding (including fibroids, iatrogenic causes and endometrial hyperplasia) and Novitas will approve it for Medicare beneficiaries with endometrial hyperplasia without atypia as a non-contraceptive therapy. Medicare does not cover contraceptive devices so CPT 58300 is non-covered/auto-denied; providers must bill 58999 (unlisted procedure) with the appropriate endometrial-hyperplasia diagnosis and include “hormone IUD for endometrial hyperplasia” in item 19 of the CMS‑1500 (diagnosis must accurately describe the treated condition).
