Skin substitute grafts/CTPs (non-autologous human, xenograft, cellular/acellular and biologic sheet scaffolds) for diabetic foot and venous leg ulcers are billable per the LCD, whereas non‑graft wound dressings and injected skin substitutes are excluded/not separately payable and must not be billed with graft/CTP application codes. Key requirements: removal/cleansing/prep are included in application codes, CPT 97602 must not be reported with graft application, E/M may only be billed if a separate distinct service was provided, and use modifier GY (Part A) or GY/GZ (Part B) for non‑covered services—see the LCD for medical‑necessity criteria.
