Billing and Coding: Skin Substitutes Grafts/Cellular Tissue-Based Products for the Treatment of Diabetic Foot Ulcers and Venous Leg Ulcers

This policy provides billing/coding guidance for skin substitute grafts/CTP used to treat diabetic foot and venous leg ulcers (including non‑autologous human, xenograft and biological sheet products) and excludes non‑graft wound dressings and injected skin substitutes as separately payable items (they are bundled into standard management and should not be billed with graft application codes). Key requirements: follow the LCD for medical‑necessity, removal/simple cleansing and prep are included in application codes, never report CPT 97602 with graft application codes, report E/M only if a separate/distinct service was provided, and use GY/GZ modifiers when billing non‑covered services.
