MolDX defines a molecular 'panel' as any ordered service measuring more than one analyte presented as a single service, producing a single result/report, providing clinical value only when combined, or measuring analytes in parallel regardless of internal lab methods. Single-analyte tests defined by a single CPT code that alone provide necessary clinical information are not panels; multianalyte services without a specific panel CPT should not be unbundled and should be billed with appropriate NOC codes (81479, 81599, 87999). Documentation should reflect that multiple analytes were performed together and that the report provides an aggregated interpretation consistent with panel billing, and NCCI/CMS guidance should be followed for coding.
