Policy Summary
MolDX defines a panel as any ordered service with more than one analyte that is marketed as a single service, yields a single result/report, is medically valuable only when multiple analytes are measured, or measures analytes in parallel. Single analyte tests (single CPT) that alone provide necessary management information are not panels, and multianalyte panels should be billed as a single service; when no specific panel CPT exists, appropriate NOC codes (81479, 81599, 87999) should be used rather than unbundling components.
