This guidance covers coding for skin substitute grafts/CTP (non‑autologous human grafts, xenografts, and biologic sheet scaffolds) for diabetic foot and venous leg ulcers and excludes non‑graft wound dressings and injected skin substitutes, which are bundled into standard wound management and not separately payable. Key requirements: use GY (Part A) or GY/GZ (Part B) modifiers for non‑covered services, do not report non‑graft dressing codes or CPT 97602 with skin substitute graft/CTP and HCPCS application codes, and removal of existing grafts plus simple cleansing/surgical prep are included in the application codes.
