Covers billing/coding for skin substitute grafts/CTPs (non‑autologous human, xenograft, cellular/acellular sheet products) for diabetic foot and venous leg ulcers and excludes separate billing for non‑graft wound dressings or injected skin substitutes (these are bundled/non‑payable); non‑covered services must be reported with GY (Part A) or GY/GZ (Part B) modifiers. Key requirements: removal of a current graft and simple cleansing/prep are included in the graft/CTP application codes, CPT 97602 (active wound management) must not be reported with graft/CTP codes, and E/M services are billable only if a separately identifiable service beyond the skin replacement therapy is provided.
