The LCD DL35041 covers application of non‑autologous skin substitute grafts/CTPs (homografts, allografts, xenografts and biological sheet scaffolds) for diabetic foot and venous leg ulcers and excludes separate payment for non‑graft wound dressings, injected skin substitutes, and other products that are bundled into standard wound management. Key coding requirements: follow the LCD’s reasonable/necessary criteria, do not report skin substitute application codes with CPT 97602 or with non‑graft/injected products, graft removal/cleansing/prep are included in the application code, an E/M may only be billed if a distinct separate service was provided, and use proper modifiers when billing non‑covered services.
