Billing and Coding: Cryoneurolysis Instructions

Noridian instructs that cryoneurolysis should be billed using temporary CPT codes 0440T, 0441T, or 0442T (selected by anatomic site) until a permanent CPT code is assigned; 0441T is the appropriate code for the Iovera system used in the knee. CPT codes 64640 and 64624 are not appropriate for cryoneurolysis because they require destructive neurolysis. Claims must include documentation supporting medical necessity and Noridian may request additional documentation and case-by-case review.
