This guidance covers billing/coding for skin substitute grafts/CTP for diabetic foot and venous leg ulcers (including non‑autologous human grafts, xenografts, and biological sheet scaffolds) and excludes non‑graft wound dressings and injected substitutes which are bundled and not separately payable. Key requirements: do not report skin substitute application codes with CPT 97602 or separate HCPCS for non‑graft products, removal/simple wound prep is included in the application code, report E/M only if a distinct service was provided, and follow the LCD’s reasonable/necessary criteria and use appropriate modifiers for non‑covered services.
