Billing and Coding: Magnetic Resonance Image Guided High Intensity Focused Ultrasound (MRgFUS) for Tremor

This policy provides billing and coding guidance for MRgFUS for tremor: services must be billed with CPT 0398T and a valid ICD-10-CM diagnosis that best describes the tremor. CPT 0398T is inclusive of radiological services (do not bill radiology separately), co-surgery billing is not allowed, and coverage is limited to hospitals or hospital outpatient departments; verify applicable NCCI/OPPS edits and include referring/ordering physician name and NPI when required.
