Billing and Coding: MolDX: Molecular Testing for Identification and Management of Hereditary Transthyretin Amyloidosis

Coverage follows the Local Coverage Determination L39935 for molecular testing to identify and manage hereditary transthyretin (TTR) amyloidosis. Billing requirements: submit the appropriate CPT and ICD-10-CM codes, include the DEX Z‑Code adjacent to the CPT in specified Part A/Part B claim fields, report only one test per patient with one unit of service, and do not add extra characters to the SV101‑7 DEX Z‑Code field.
