Billing and Coding: MolDX: Molecular Testing for Identification and Management of Hereditary Transthyretin Amyloidosis

This billing/coding guideline supplements MolDX LCD L39940 and provides administrative requirements for molecular testing for hereditary transthyretin amyloidosis: select the appropriate CPT and ICD-10-CM codes, include the DEX Z-Code adjacent to the CPT code on specific claim fields, and enter one unit of service. Only one test may be billed per patient. Follow LCD L39940 for clinical coverage and medical necessity criteria.
