Policy Summary
Non-invasive FFRct is covered only as post-processing of a previously performed CCTA for patients with documented symptoms consistent with stable ischemic heart disease and with clinical findings that justified the initial CCTA. Claims must include the CCTA report, FFRct analysis report, BMI, documentation of symptoms and clinical findings, and the referring/ordering physician's name and NPI; coding must comply with NCCI and OPPS rules and an ABN may be used if non-coverage is likely.
