Covers CPT 95165 for the physician’s preparation and provision of allergen antigen extracts (non‑venom) including concentration/volume calculations, but excludes the administration of the immunotherapy. Key requirements: the number of doses must be specified and reported by antigen type, vial source is irrelevant, and billing is limited to a maximum of 10 doses per multidose vial (PE based on a 10‑cc vial).
